How to register
If you are new to the area and wish to register, please call into the surgery or fill in the application forms below. Once these are completed and returned to us your registration will be considered. To register you will need to provide us with your NHS number, full address including postal code, telephone number, a form of photo identification and proof of address. For children we wil need to see their red book, birth certificate or passport. Please note to be eligible for registration you must live within the Practice area (see catchment area). If you are prescribed medication by your previous GP and need to continue this, you may need an appointment to see your new GP to enable him/her to continue to prescribe your medication. Please supply a list of your repeat medication and a member of the team will advise if you need an appointment once registration is complete.
The government recently passed a law to change the rules for organ donation in England from 2020. The law introduced a system commonly called “opt-out” or “deemed consent”.
Everyone in England over the age of 18 will be considered to be in favour of donating their organs and tissues after death unless:
- they have opted out
- they have appointed a representative to decide for them after their death
- they are in one of the excluded groups – under the age of 18, ordinarily resident in England for less than 12 months before their death, or lack mental capacity for a significant period before their death

Temporary/Short term resident
We provide a temporary patient service for anyone staying within our Practice area e.g whilst on holiday, staying with relatives etc. provided the friend/relative registered is not within a reasonable travelling distance. Just call into the surgery to complete a Temporary resident form, identification and your NHS number will be required.
Overseas visitors can register as temporary patients. Entitlement to free, immediate and necessary care is dependant on eligibility, please ask reception staff for more details.
